In December of 2012, the Vermont Council on Homelessness approved Vermont’s Plan to End Homelessness.  To make progress on the plan’s goal of expanding the capacity of programs to provide voluntary supportive servicese once households move into permanent housing, the Council is collecting information through on-line surveys from housing providers, clients, and tenants.

The surveys are intended to help the Council’s Supportive Housing Workgroup understand and assess the issues and challenges, both for clients/tenants and for housing providers.

Complete the survey by clicking on one of the following links:

ALL SURVEYS MUST BE COMPLETED BY NOVEMBER 14th, 2013.

If you are a housing provider, in addition to taking the survey yourself,  please assist your permanently housed client in participating in the tenant/client  survey (i.e. you may print it out and then enter the responses online later, or assist the client in accessing internet themselves, etc.)  A second follow-up survey will be circulated for housing services provideers in December.
